About Transitions
Transitions started in 2008 when Dr. Flynn began treating people with “opioid addiction," with buprenorphine in the Saturday free clinic supported by UC Davis School of Medicine called the Joan Viteri Memorial Clinic (JVMC). As the number of patients being treated increased, he could no longer see them all only at JVMC and without additional help. In 2011 he joined with Harm Reduction Services to form the Priorities Clinic. After two years, Priorities Clinic changed their affiliation to SANE, another non-profit organization in Sacramento.
Where It Started
After two years with SANE and growing to about 150 patients, Priorities Clinic became independent as Transitions Clinic. While we have remained in the same building, we have grown throughout that time, now serving over 500 patients regularly and expanded services. In 2022, we became a non-profit organization and have since begun accepting insurance, including Medi-Cal.

We Strive to Provide the Best Addiction Care
We provide a full range of addiction medicine treatment regardless of the substance, including medications for addiction treatment (MAT), as well as counseling and mental health services. We lead with a harm reduction mindset and strive to provide the best, most affordable, complete addiction care to all of our patients.
